That's normal behaviour for the game. You can't combine the same effect from different sources.

Afaik you can combine items/spells of the same effect but only the highest one gets applied. Am I wrong?

In the SRD/OGL you are right. But the game is not an exact adaptation. In the game you can't do that.

Fair enough, there are certainly more important matters to attend to.
But maybe - only maybe - you'll keep it in mind and one day...

I just noticed that you don't seem to be able to reload a heavy crossbow while slowed. Intended?

Not intended - you should be able to reload as a standard action when your round is restricted to a standard action. Goes to the list of things to fix.

Side note: why is there no spell/item to increase natural armor? Barkskin/Spiderskin or Amulet of Natural Armor is part of my AC optimizing plan...

No there are no robes, but the wizard can put on cloaks and bracers of armour. Or a light armour.

Nifty request: I want robe! I don't even demand the Robe of Vecna, let's say something humble like a Robe of Minor Fire Resistance, you get the idea.

Mithral leather armor is as close as you'll get (0% spell failure). A solid enough replacement for mages, but you'll still be without armor suited for a dex tank as it's fairly easy to break the 26 dex mark (Hell, you could do it at level 1 with the +6 dex bracers). Sure, they could go without any armor, but then you'd be giving up the ten enchantment points that you could fit on the armor.
